took another look at your log, you can see the ecu is calling for different injector duty cycles but the pulse width is stuck at that 1.28 value, which again im guessing is a hard limit that you will need to lower.
*** IGNORE this, its too late for me to be posting lol. Duty cycle is just a value calculated within evoscan and is not reflecting what i thought it was.

Bit of an update! Made some progress on this. Found out my wideband sensor was bad. Turns out that 14.7AFR is around 12.0 on the gauge so it's running super lean. Richening up the mixture got rid of the issue at first startup; but after the car warms up it falls on it's face. Something is being triggered in the ECU that's causing the issue. It's obvious if you look at the log.
Goes from a perfect 13.8AFR at idle to 17.0AFR! It falls off a cliff there. Really weird. Hoping some of the guys over at Lancerregister (or here) who know the Evo 5/6 ECU well can help me figure out what it is that's causing the issue.
