Unhappy Why is my clutch slipping?

I wanted to ask this question because last week my clutch went from being fine and never slipping to wham slipping in every gear under acceleration.

This is what happend: I was at a light. I eased the car into first. Once I was in gear I floored it to redline. When I shifted into second it felt like the clutch pedel stayed on the floor? and the revs went up.

I pulled over to check things out. The clutch enguages right off the floor and it slips under acceleration right around 3000RPM's in every gear.

Is the clutch gone? I thought that it would start to slip slowley at first. Do I need to adjust the pedel? if so is it easy to do?

I have a 04 EVO with 8900 miles on it. I have a bunch of mods I hope right around 300HP. I don't launch the car often. just drive it hard.

Thanks for the help.

The reason it happens so rapidly is that there is a stop built into the pressure plate that keeps it from pressing on the disc once it reaches a certain thickness. So once the disc is worn to a certain thickness, the clutch starts slipping badly. One day it's fine, the next day it slips badly, the following day you are walking! Here is a photo of what I am talking about.

I got an ACT clutch and like it very much.
In my case with over 25k on the car, it only started slipping on the race gas map at peak torque. It only slipped like 200-300rpm, but the amount of torque the modifications were making was overwhelming the stock pressure plate. Everything that came out was in great shape, just not physically up to the task. The new ACT holds great on race gas. Great value!

The clutch was just starting to hit the stopper so it was tost. One other thing that the dealer said was that the actuator mounting block was cocked/ lose they had to fix/tighten it. I think that was why the pedel feel went with just one shift and all of a sudden started slipping.
