Exedy Wave Clip or Master/Slave Cylinder?

So I just had a question since I haven't really gotten a good look at my car. Here's what happened:

There's about 1700mi on my car since it was fully rebuilt by a shop and I had them reuse my Exedy twin with which I've never had any issues with. Started out in first and gave it the beans, tried NLTS around 6500RPM and it went into second just fine. I then accelerated in second gear fine for about 1-1.5 seconds and then the clutch went straight to the floor while I was accelerating. My foot was not on the clutch when this happened, it was in gear, and it was accelerating.

So to those that are familiar with clutches, does this sound like a wave clip popping off or does this sound like an issue along the clutch line (master or slave)?

My guess is that the wedge collar wire ring became dislodged during the shift and when you lifted you foot off the clutch pedal, the TOB had a chance to work it's way completely out of the ring. If that's the case, I think we have a fix for that:
