Here is a video of my worst run by far, on my most powerful setup. 
This is what happens when you launch with the stock clutch with some decent power. I remember my first pass some 3 1/2 years ago, I ran a 14.2 in complete stock form. This pass was a 14.2 at 109.

Haha, yeah everybody was like "you will be fine for 1 pass." I was telling them no, I really don't think so when in reality, I knew that once I launched it the clutch was going to be like F you Nick, and spin the rest of the way. Sure enough, that's exactly what it did as you can see in the video.
I've only had three passes ever and I was fortunate enough to do okay. My worst was a 12.9 @ 106 1.78 60' That was with 320whp/wtq so it definately wasn't one of the better times for my power.

12.79 @ 111.14 Nice Trap, Qualify'd for Quick 8 (Yipee, this would end up earning me FRU + $50.00)
13.68 @ 94.96 First Round Win (heads up, lost ET and Trap but crossed finish first - somehow)
15.55 @ 64.93 2nd round Car doesnt make it to the line. I'm in the finals with this umm Pink car driven by a girl.
