It can happen because not all the pullies are 1:1 ratio. the cam gears and crank are 2:1 the cams are only TDC every other time that the crank it TDC. The oil pump is 4:1 I think. That means that it is TDC 4 times for every one time the crank is TDC. In theory you could be at 90 deg with the crank and set the oil pump at TDC and it wil line up at crank TDC. Everything is in refence to the crank cycle. It is a very delicate process.

Thats where checking everything comes into play. Sure on the 4g (compared to the Honda's) you have to roll it around more times...but I'd rather be safe and sure than sorry.
Yes your balance shaft is 180 degrees out of phase. Don't listen to these other people saying that it can't be because of the gear ratio and so forth. If they can't get a screwdriver into the hole to check the bs position, turn the timing mark stright up at the 12'oclock position (with the belt off of course), if it feels that it is wanting to roll towards the mark, it is in phase, if it wants to roll away from the mark, you need to turn it 180 and then you will be good. this is why i eliminate the BS's out of all my cars... lol your oil pump BS is probably the culprit if everything "looks" right. trust these guys^, they know whats up.
