clutch will not disengage

I was doing a 3rd gear pull today while I was tuning, and I left of the gas and pushed the clutch in. After that It would not o into any gear. The car shifts fine when off. I tried to start the car in 1st gear with the clutch in and it lurched forward. The TOB is still attached to the pressure plate and when the clutch pedal is pressed in the pressure plate does move back with the fork. I am running an ACT XTSS with a 6 puck solid hub disk. Is my clutch toast? It didnt show any signs of slipping at all.

Is that a new TOB when you replaced the clutch?

Yes it was new when in installed it 2 years ago, after looking at it closer it looks like the TOB is not sitting against the clip correctly. I tried pushing it into the pressure plate after removing the slave cylinder and it wont go in.

sounds identical to me...

here is what i found with my sprung 6 puck





the spring & other bits were jammed between the pressure plate/flywheel/failed disc
