I got me a optomitrist to write me a prescription type note and included his biz cards to make it more authentic. In my state you cant have the front windows tints. If you wear glasses you might be able to get one of these if you can pursuade your optomistrist.

Do you mean you can't tint the front side windows or can't put that 5in strip of tint at the top of the windshield? Either way I don't see why they make a big deal out of it as long as your not tinting the side windows 2% which then becomes a problem for police who might pull you over.

City of Chicago is doing anything to write/give tickets for anything now thanks to the Mayor trying to raise more funds for this city. Besides If I do get pulled over, it would feel good being legit with this paper

Keep in mind that even with a letter from the tint shop and something signed by your MVA/DMV a cop might still give you a ticket and make you go to court to prove its legal. It happened to a friend of mine. He had to miss work to go to court to show a judge the paperwork so it could be tossed. Fine use of a states resources.
