Steering Cluncking Noise

I have an 03 with 16k miles, I found cluncking noise when I start truning the steering wheel. If you check the steering play and you can feel the clucking noise. Actually you can feel it if you put your hand on the tire/wheel and rock the steering wheel left and right.
Anyone have this issue?
Sign of bad tie-rod end?
