P2096 code 2015 EVO X MR

Having issues getting rid of P2096 code.

Car is on a 2015 MR Cobb Stage 3 OTS Map (ACN91/Normal Wastgate)

Current mods:

Cobb Accessport V3 AP3-MIT002
Cobb Catted Test Pipe
Cobb DownPipe
Cobb SF Airbox Kit
Cobb SF Intake System
Cobb 3 Port Boost Control Solenoid
Cobb Turbo Inlet
ETS 3.5 Front Mount
ETS Upper and Lower Intercooler Piping
Buschur Racing Evo X S.S. Cross Flow Catback Exhaust

I have all Remflex 2-bolt exhaust gaskets installed and Vibrant 11619 J-Style Oxygen Sensor Restrictor Fitting with Adjustable Gas Flow Inserts. (Currently have medium sized insert Installed).

New Rear Sensor from Walker Products 250-24751 4-Wire Oxygen Sensor (Installed in July of this year).

My OEM sensor was stripped thus the Walker was installed in its place. It seemed to hold up for a 1,000 -1,500 miles with no issues.

Nothing on the car has been changed since than.

I've checked for exhaust leaks and I found nothing.

Now the P2096 comes on at every 150-200 miles.

I always clear it immediately when it comes on.

I was reading in many year old posts after 5 ignition cycles it goes away, not sure if that's true or not and yet to try it.

Also read from old posts that the spacers are hit and miss with the EVO X....

In addition some say to plug in the O2 sensor directly to the test pipe with no spacer etc. yet to try that either.

I won't have the luxury to be protuned anytime soon.

Wanted to know if their is anything else I could try or implement to resolve this issue.

Any input would be highly appreciated.

I know this isnt the input you're looking for, but I installed a HFC this weekend that I was promised "doesn't throw a CEL" because of the spacer the pushes the rear o2 out, yet low and behold got the same CEL. Reset it, drove to work and back, came right back on. I don't have a full exhaust so the gains are minimal, just gonna take it back off and throw it on for sale locally. These cars are SUPER touchy with the rear o2. Most build a tune that doesn't throw a CEL for that rear o2. Sorry

I have a high flow catted mid pipe with no spacer. The accessport custom tune did not eliminate the code. I found in hot weather, high traffic conditions driving in 2nd or third gear for prolonged time threw the code. Hit or miss but has been miss for the last few months. Clear it and keep on driving.

Installed new Denso o2 Sensor directly to the testpipe and installed two new GrimmSpeed 3" Gasket 2X thick. In addition, I cleaned the map sensor with CRC and I found a small exhaust leak in the rear 3" exhaust clamp that was also contained. I'm close to 300 miles and yet have a SES light.
