This is a Cobb Accessport V2 for an Evo X. The second photo shows it is un-paired and ready to install. It is in pretty good shape, it has some scratches on the case's paint but is otherwise functionally perfect. I'm selling this because I sold my Evo X and obviously don't need it anymore.
