I have the drivers side headlamp assembly for sale in mint condition. They have never been opened and do not come with the Ballasts or bulbs.
I also have a passenger side for sale but the center bracket is broken.

Here you can see the tab that is broken off the passenger assembly, as you can see, it will not effect the durability of the headlight as there are still 3 other mounting points. Also, this can be fixed with epoxy or simply plastic welded back together.
