Used SAFC II with harness (No patch harness, but you can always buy one and solder away). I used this for about 10 months. $180 shipped. I peeled the old adhesive off so you will just have to buy some adhesive tape and the bracket that is attached will work just dandy.

It's blue screen and silver outline. I thought they were all the same.

As far as the field harness goes, they don't usually come with those. Mine was soldered right into my ECU harness, which is what most people do. I admit that it left a mess when I removed it. I would recommend the field harness, but that is usually separate anyway.
