That smiley is mimicing "rolling on the floor laughing" commonly abbreviated "rotfl" or "rofl" and that smiley is usually linked to the vB code ":rofl:" Granted, as a smiley, it doesn't have a body below its head and arm so it's a bit difficult to tell what exactly it's doing.

Just thought you should know.

When you added it to your smiley list you were probably wondering why this smiley that you thought was picking its teeth was linked to "rofl" on the site you found it on and now you know.
