Okay, this is what is going on now. The car was reset (unplugged the battery and turned the lights on for like ten minutes) and no change. Welded everything up again on my manifold and have ZERO exhaust leaks. It still reads all the same. Leans out WITH the o2 and rich WITHOUT o2. I am going to reflash the car back to stock and see if it would like the stock ROM better (for some reason) since there really isn't a lot without boost that's changed in the ROM and see if the car will run part throttle (no boost) on that tune. If it does I will flash the other back into the car and see if I get a change (stupid I know but what the hell?). Then I will change the o2 (since it seems like it is a nightmare as I have stared at this **** for hours and tried to get it out with no avail period). I will get this fixed I am sure and I will post my results. After the o2 gets replaced I will buy a temperature sensor I suppose. But everything is pointing at the o2 in my book still.
EDIT: I have also looked at all my charge pipes again... all tight. I have looked around for vacuum leaks... looks okay but I will buy a cigar tomorrow and blow smoke through a straw everywhere in the engine bay. I used a candle to find where the exhaust leak was when it blew out, fixed that and now everything is good there. I took a high powered light and looked at the o-rings on the injectors... no leaks. SO that's the run down thus far.

Today before I got the o2 out of the car I decided I'd be less lazy and run a log on the car with evoscan. I turned on the o2 readout in the log. The readout showed the o2 to be running 4.9776 for a while then the o2 started putting up numbers such as 0.2436 and 0.3537 instead of the normal 5. That STILL points me to o2. Will report back when changed.
the o2 sensor has oscillating voltage. it should go up and down while idling or under a constant load, out of boost. if you are idling or cruising down the road in vacuum and a constant load AND the sensor is only putting out a steady voltage, it's probably bad.
assuming EvoScan is set up to read it correctly; i've never logged o2.

Then wtf could be wrong with this car... Just out of nowhere this started happening. Idk what to do. I am fixing to go buy a pack of those 99 cent cigars to smoke to puff into the vacuum lines to see if it could be vacuum related here in a sec.
Can you swap coolant temp sensors with anyone? I dunno that's the issue....but I'm stuck on that after having a similar issue on another car..

Just pulled obd codes out of the car. I erased the previous then ran the car again with the o2 plugged up and got: p0132 0138 0141 0507
it's screwed right into the thermostat housing, front the front. super easy to get to. i THINK it has a single-wire running to the plug. maybe yours is unplugged. or, to make it easy, you can just log the coolant temps and see if they are normal.

I always log temp. It is normal. Seems to be working properly all the way down and up.

Lol I'm pretty sure possession isn't the answer. Since I'm pulling o2 sensor codes out of the car that SHOULD be replaced tonight. The IAC threw a code as well but it was for unexpectedly high rpm idle which was the result of being way way too rich. So I'll continue to see what can be done.
