ACT Man, need your help!!

What would be the only reason you would need to replace just the disk in a clutch? My clutch is slipping only when I roll into it on higher gears and the clutch is pretty new. 9 months old and less 6k on it. i have tried adjusting it but to know avail.

Thank you

The disc has worn to the point that the pressure plate is not clamping on it any longer so the clutch slips.
