lockout happened from the very first day of driving. had to continously adjust the clutch pedal higher and higher

The clutch disc might have gotten tweaked if you:
a) Let the transmission hang by the input shaft.
b) Used a floor jack to install the transmission.
The weight of the EVO transmission is enough that if you let it hang by the input shaft or if you have to wrestle the transmission to get it to go on or ended up jacking the thing around or raised the engine up while using a floor jack there is a good chance that it'll bend the disc.
I've have had several different clutch discs sent in over the years and have seen clutch discs that have been tweaked out of true by as much as .045" and when this happens there isn't much one can do about it other than replace the clutch disc. Typical run-out of a clutch disc should be less than .010"
The pressure plate can only move so far and if the disc is more out of true than what the plate can move problems can arise.

the trans was installed using a hoist and 2 people. we even removed the subframe to make sure the trans went in straight, and it also made the install process easier. i will be removing the trans again sometime next month for you to look at the 4th gear grind. i can also bring the clutch so you can inspect that as well.
change your front motor mount or install a dampener. If in fact you have the clutch set right, your engine is moving too much during hard shifting. How many miles on the motor mounts and what rpm do you experience lockout?

i have a forge front engine mount. the car has less than 20k. i had experienced locked at about 7k and above. then i adjusted the clutch pedeal even higher, and didn't experience lockout. then about 2 weeks ago, i started to develope 4th gear grind. the tranny and oil have less than 1.5k miles on them
if the clutch is adjusted, you have low mileage on the mounts, then I'd say you warped something along the way due to excessive heat. Either the flywheel, the pressure plate, or you slipped the clutch enough to develop high spots on the clutch facing by basically melting the friction material and redepositing that material somewhere else on the disc facing.

i used penzoil syncromesh
if i wraped the clutch, why am i only getting grind in 4th gear. also, the clutch engages very very smoothly from a stop. no shuddering or vibrating, the symptoms a wraped flywheel/pressure plate would have.

well, i plan to be taking out the trans one more time and taking it back to TRE. i hope he will fix the 4th gear grind under his 180 day warranty. part of the repair, a new 4th gear was installed, so i hope the grind hasn't worn out that gear a lot.
right now, i really really take my time shifting in 4th. if i hit 3rd gear at 7400 rpms, i wait till it drops till about 6500 rpms, and gently guide the shifter into 4th with my fingers.
