clutch lock ring will not stay together

clutch lock ring will not stay together
I have changed the clutch 2 times in the last week because the locking ring for the pressure plate has came out. what will that be caused by is something messed up in the trans or the shifter fork? i have used two diff pressure plats and locking rings with the same results. please help 
who installed the clutch? if you bump the retainer with the input shaft of the trans, it will pop off. if even one of the hooks holding the ring in place are bumped off the ring, it is certain to pop off.
