P0125
UPDATE !
we checked all the connections and everything was normal. Put a new rear 02 sensor and front and still the same. Finally it made us believe the problem was with the tune.
We hooked the car to the dealers mut II computer and it showed a reading of '0' for the rear 02 sensor. which was weird. we then put back the stock injectors and previous flash and the rear 02 sensor began to show a reading.
The problem was narrowed down to the flash causing the rear 02 sensor to show '0' reading...which in return is making the car behave abnomally..bad idle, hesitating, sluggish, check engine etc.
weird, the flash was causing the rear 02 sensor to be disabled somehow. talked to my tuner waiting for his input..anyone encountered such a thing? it's really puzzling how the flash was doing this.

I have a bit different but very close problem. Car also goes very slow and has bad idle. From the loggers I see front O2 sensor shows morbid values. Around 4.5 V instead of 0..1 V. However physically attached tester shows correct values 0..1 V so it makes me assume it's ECU "thinks" or somehow interprets the correct signal as 4.5 V. Also car CELs P0125 all the time... I have default values for:
ECU Periphery0 = 0x56DA
and also default value for:
Min temp for closed loop = 0
Can anybody suggest me what to do?

did you recently flash ur ecu? try putting back ur old tune and see if that helps.
My problem was not related to what the code suggests at all (bad thermostat, coolant level, bad coolant temp sensor). I narrowed it down to the tune itself when i flashed it.
I got another flash from another tuner and no more p125 !

The problem is I don't have old ROM image... I'll get it soon and will give it a try however I'm not sure it will help as I have 272 camshafts installed so I'll have to re-work the settings... But I'll check for O2 sensor reading of course.
In my case I have 1) Closed loop temp set to zero (so P0125 at 82 degrees C is an absolute nonsense) and 2) I have O2 front and rear sensors ENABLED.
