Any idea why this occurred, the stock shortblock had never been apart and wondering if anyone has any input on how to prevent it in the future. There was a lot of debris behind the timing belt cover due to the sprocket and pulley having severe wear.

I found the same problem today with mine. One of the pulley bolts broke off in the sprocket. All four holes in the pulley were stretched because of this so I had to order a new pulley all together.
Well all of my bolts were in there but they weren't tight due to so much wear taking place, I am sure in a few more miles mine would of crapped all together.
I don't know how to prevent it honestly. My sprocket didn't show any signs of damage, but I went ahead and retapped the holes. If this happens again, I'm going with an aftermarket pulley for sure.
