Cold start problem - MR
Hmmm, I found that if I press gas pedal during cranking, the car holds 1200-1500 RPM after start nicely (no hesitation between 400-1000 rpm like before).
It looks like the throttle plate is too closed during cranking on idle. But why???

Maybe a comparison of the throttle tables to USDM cars would help you out?
FYI, one word of caution... in my experience with this table when I set it lower than stock I am fine on startup, but it exacerbates my lean condition for the first few seconds I'm on the throttle (which makes the car fall flat on its face). If I remember correctly it did the same on my IX, but not everyone I've adjusted it for has had the same experience.
If it works for you, then I'm all for it!

Well, it looks like I have found a problem (which may cause the starting hassitation):
I meassured intake and exhaust valves clearance and I found out that 10 of 16 have NO CLEARANCE! and the rest has smaller than prescribed. It looks like some of them are partially opened during cold startup.
I will have to set them all properly and let you know if this was the issue.

Cold start problem - MR - SOLVED
I found negative valve clearance at 6 intake valves, after setting all valves to proper clearance my problems are gone and car runs like new from factory :-).
