I have a used Exedy triple clutch kit with the HD pressure plate, part number MM023HR, that has been used for about 3,500 miles since upgrade. It was originally the twin clutch with the SD pressure plate, but when I sent it into Exedy last year, it was upgraded to the HD diaphragm. Also three disks and three pressure plates, with the hub, were purchased new. When it was at Exedy, the whole assembly was rebalanced, and you can the punch marks for the proper alignment. Two of the disks measure 3.95mm, while one measures 3.90mm. Brand new is 4mm, and replace under 3mm. The flywheel was resurfaced with a step height of 32mm +/- 0.01. Exedy also installed a new throw out bearing collar, and I added an extra wave spring, for a total of three. This will come with an STM clutch fork stop and a 3,500 mile used Exedy throw out bearing (Not shown in the picture).

As you can see from the pictures, the cover is dinged up a little from the throw out bearing bolt coming loose, but it was not affect the functionality of the cover.

Also, The spline hub goes in with the lip toward the flywheel so it sits on top disc closest to the flywheel.

also, i am willing to separate the the disks and plates for anyone who wants to convert their twin disk to a triple disk setup.
You will get the 3 disks, center hub, and two center plates for $750. This setup is $1050 brand new.
Add $120 for the front pressure plate.
The pressure plate for the twin and triple are the same, but you have to make sure yours is not warped.
