I havent come across one that has failed, but if it were to fail it would result in a Check engine light, = Throttle Voltage low. The only time I have seen this code come up is if someone doesnt click it in all the way. When you start the car it will idle really really funny.
In terms of driving, its either on, or not working at all. The throttle system uses less than one volt with our model vehicle so the odds of it cooking itself is not really possible unless you play with other sub components around it.
If you dont think its working, the best thing to do is to take the SB out. Once you do, it will be a clear as day difference going from SB to OEM. I have done it twice lending it to friends, and its hell
