Our current design can't come off, we have a bracket that bolts to the frame preventing the thing from getting air time in case it comes loose from the coupler.

I have t-bolts on but maybe not tight enough. No further problems since.

I have had that happen too man, fixed it and was out running about 20 minutes later. Mine happened going through the traps and not on the launch. First time it happens it is scary though
