Do you guys agree that its probably warped disks? I only had this clutch on for 14k miles. Could this have been instigated by the improper clutch adjustment for those past 14k miles?
It could be warped disks, which is usually caused by excessive heat. That excessive heat could be caused by the clutch constantly slipping rather than disengaging every time you push the clutch pedal. It could also be that it needs a little more adjustment.

I would try changing the transmission fluid.
also adjust your master cylinder all the way down where the cylinder rod is barely holding onto the U bracket.
also the bracket needs 13mm wrench and 11mm (for the inside screw). you have to re-position the little white button box. dont worry, it wont brake if you move it. clutch in sensor is what i call it.

I already replaced my tranny fluid with Pennzoil+adjusted clutch pedal. I looking for a good shop to take it into as I am all thumbs. Mistuorder do you know how to adjust the shift linkage wire as plokivos suggested?
I've heard of people cutting them and then recrimping them but I really wouldn't suggest that. Other than that method I have really never heard of this. I'm just thinking it's your clutch man. Let me know what you want to do. Oh hey a friend's shop here in New Braunfels is having a German car dyno day tomorrow and we'll be making the jig for that pipe I was telling you about using my car tomorrow morning as well if all goes as planned. IMO, All of ur efforts are no use, Mitsu trans is just pice of garbage. take that to sheppared racing or somebody and custom build it. My brand new 05 evo (1000 miles only) grids 5th gear with stock clutch when I drive hard. sometimes, 2nd gear doesn't go in. 3rd gear grinds if I shift too fast. Mitsu trans is not designed for fast shifting. you have to shift like ur grand pa.

Not to scare, but Ive seen and heard of 3 different HKS clutches $hiting the bed, causing this issue.
As far as adjusting the clutch, the harder way is to remove the pin. Get back there with a 12MM wrench, push the wrench to the left and it'll break that nut loose. Then turn the rod and you'll see it turn. This problem doesn't sound like that, however. Best of luck.
BTW I have a cusco twin disk and this problem is quite common on 2 disk setups.

Well, yeah, its noisey on a downshift and the T/O bearing just started making noise. The adjustment has been done several times and it has come to the point where Im gonna either splurge for an RPS carbon/carbon or go to an ACT single. The clutch just doesn't disengage right and I dont know what else I can do to make it.
I had an issue with a twin disc, warped, and also had an issue with the factory clutch line breaking at the "nipple" causing a minute leak, and allowing air to get in, dont know which one it was, replaced both at the same time, had to tow the car in 3 times for this issue, check the line behind and under the airbox for a leak. (run your finger up the line) Had to get a "special" bracket made so the Nipple would'nt crack and cause the influx of air. Also check the brake pad wear,a nd the fluid level. re-bleed the whole system a couple times. My 2c. Good luck.
Holy BeJesus!!!! All the things I brainstormed and I didn't even think of that ONCE!! Man oh MAN! It seemed like an issue thats progressive and hardly faulty product related. I'm gonna tear down the whole hydraulic system tommorow and go from there. Good idea, smokedmustang...I feel stupid.

Not even for second passing judgement on the weakest link....second to only ME, the hydraulics.
