I don't know dude, that looks to me like the hole is at the end of a bead from a welder. Has someone been welding on your manifold? The picture is pretty fuzzy though.

Its either a bad weld or a defect in the metal, you are just incredibly unlucky, they should replace it under warranty.

replacement is the least the dealership should do. you deserve some explaining, and a lot of new stuff for free.

seeing as the factory manifold is cast, you'd probably be hard pressed to find a weld on it. that said, it does look like there is a small weld in that location. a better pic would be nice

also, the factory manifold would blow out the freeze/machining plugs long before it took any damage

this is most likely a defect in the casting & an isolated incident.

ahah that's crazy, I wonder what mitsubishi would say about it when you take it in, I bet they'd be scratching their heads, but that's definately covered under warranty, tell them that you want it completely replaced because I'm sure they would want to take the easy way out of just re welding that portion
