symptoms of a warn clutch?

Hey im new to driving stick so dont flame me to hard, but i recently bought an evo, and its been great untill last night, and im trying to see if i can diagnose the problem with some help from more knowledgeable people. Ive noticed as time went on, that the shift lever went from notching into the gears to sort of sliding into them(so basically stiff to weak) and when its in first sometimes it jumps out into neutral, when i bought it i was told it had an exedy 3 puck in it, because it has mods, so is my clutch burnt or is something else messed up, any help would be great oh by the way when i would downshift to first it would make a grinding noise even with my foot on the clutch- heres some mods if that makes a diff with anything

272 cams
750cc injectors
fp white turbo
and other supporting mods

finished clutch.... 4th gear, floor it, RPM's shoot up and your car does not go = slipping clutch

if its popping out of gear, you have worn syncros and need a trans rebuild-- should have asked the last owner why he replaced the clutch, could have given you insight on how it was abused before you got someone elses problem, which it sounds like you own now. Good luck with that.
