After a clutch install, vibration in clutch pedal increased more and more to an almost violent shake which caused a tear down again. Also an increased metal whine when going from a stop. Improper/uneven torqueing down the bolts of the pressure plate to the engine was the culprit.
Is this Pressure plate and flywheel show signs of being warped and needs replacement, which I'm leaning towards.
I don't know the cause but the clutch is heat spotted bad enough to require replacement of the pressure plate and replacement or turning of the flywheel.
