Buschur stage 3 install then disaster

So here's my story, I just installed a stage 3 head on my evo and got the car tuned and was on the road for two weeks. On the day she broke down I was dropping my exs little girl off at school. Was driving in the parking lot and went to shift from first to second and the car died. It was a soft stall n was occupied at the moment so figured I miss shifted or something and that was my mistake turning the motor over after the stall. Upon inspection the teeth of the timing belt were ripped off the belt where the crank met the belt and bent all the exhaust valves along with breaking all exhaust roller rockers.

The exhaust camshaft spins freely and the intake I can move with a wrench but still in contact with valves so can't turn all the way. I have no idea what could have caused this. The car was running amazing with no red flags. Oil, boost, knock, egt everything was spot on. Belt was a brand new napa premium belt. I'm afraid to fix everything just to have it happen again... I need to figure it out or it's just going to happen again...

I would say the belt or belt tensioner failed. Did you replace the tensioner when doing the belt?

What did you torque the cam caps to? Did you lube up the cams before install?

Did you do all the work yourself or did a shop install it for you?

the tensioner wasnt replaced so i think that mite be the problem but only got a little slack after sitting overnight and this happened after a bit of driving.

I dont remember the torque but i followed oem specs and yes i used assembly lube when installing cams.

Also i checked the oil pump gear and it spins freely ill check the balancer.

Does anyone know a way to test my old tensioner, I remember still having tension after the teeth on the belt ripped off? I already ordered a new one but would be nice to pinpoint the cause of this disaster b4 repairing everything.
