Transfer case, drive shaft issue. Common issue?

So I'm looking for 2 parts that are pictured below, but I'm also trying to figure out how they got chewed up in the first place. I'm thinking the only way this could possibly happen is if the T-case was low on fluid and started to throw pieces of metal into the fluid and found their way down to the drive shaft input housing, whatever it is called. Any other ideas? Also, is the t-case a common problem or should I just replace the end pieces and not worry about the whole t-case? Where do I find this stuff...

You can see the drive shaft end is all scored. I may be able to polish it back to normal but I'd rather get a new end.
