ACT HD Street Clutch issue?

ACT HD Street Clutch issue?
So clutch issue with the Evo:
4th gear pull to redline, when I went to shift for coasting, the pedal was vibrating. After I shifted into neutral, I had no clutch pressure (Vibrating pedal was continuing, but the pedal would go to the floor without resistance). After rolling up to a stoplight, I pumped the clutch in neutral and regained pressure and got it into first. From there I clutchless shifted until I got home, but had no clutch pressure in gear. In neutral I did have clutch pressure.
Halp?
Clutch is an ACT HD with Fidanza flywheel. 20k+ miles.
It's been hard to get into gear from stopped since I've owned it.

That pic does not look like the TOB came out of the pressure plate.

An ACT monoloc callor is much better for keep the TOB in the pressure plate. However, your TOB does not appear to have popped out. The clutch fork is pretty close to straight sticking out of the trans. When the TOB pops out, the slave cylinder fully extends, and shoves the part of the shift fork you can see all the way towards the engine.

had it actually popped out, and you kept pumping, the pedal would have gone stiff. If you had tried to pump more it likely would have blown the slave cylinder.
I had a correctly adjust stop on my car, and it still popped out on me one day. Monoloc went in upon re-install.

Update: The clutch disintegrated. A spring came off the ACT and the rivets broke.
Installed Exedy Stage 1 HD (05830HD) and a monoloc for good measure.
Now the car won't really engage 2, 4, or R. It does actually engage (90% of the time) and will drive in those gears, but it feels like it only shifts halfway into the gear. As though it's an extremely short throw or that the linkage isn't fully engaging.
When the car is off, it shifts fine. Thinking either clutch isn't disengaging (only why would that only affect the bottom gears?) or that the linkage wasn't installed properly when reassembled..
Thoughts?
