bent slave cylinder push rod
maybe something happened to the rod and clutch fork? thats all I can think of...get it in the air and see where the rod it at (look/feel on the top portion of the tranny, or you can remove your airbox to see...
Take the slave cyl back off and see if the fork moves freely back and forth, if it does then the t/o bearing came off the pressure plate. If it does not move freely then it is most likely still attached and not the problem. I still think that you don't have the hydralics bled properly. If a bubble of air gets in the master cylinder, it can be very difficult to get out without a vacuum bleeder.

Sure sounds like the t/o bearing indeed came off the pressure plate. The fork would be virtually unmovable by hand if it was still connected. The retaining clip must be broken or something. Unfortunately (or fortunately depending on how you look at it) it sounds like you will be in the market for a new clutch kit. Time to upgrade. Any shop that installs the clutch can bleed the hydralics for you too. Good luck !!
i checked and my retainer ring for TOB came off.... anyone know would it be possible to get it put back on without taking the whole transmission apart..... there is about a quarter size hole on the bottom of the bellhousing...i can see the the ring is lose....anyway i can put some tool in the hole to put it back....im really tight on time and money right now...any help would be appreciated.....
