got it! it is made by Standard Electrical, NY part number HP7310 Alt. B, 36357-321

Bringing back an old thread.. just finished this mod last night. :-) Thanks to the OP for the great write up. The fan now comes with a different harness side connector, so I needed a different connector to replace it to fit the HP7310 connector that was suggested. I had to buy this,
Amazon.com: Standard Motor Products HP7320 Connector: Automotive Amazon.com: Standard Motor Products HP7320 Connector: Automotive
HP7320 connector which will then connect to the HP7310. I couldn't find the part # for the connector that comes on the fan. :-)

I didn't use a relay, I just hooked the fan negative wire to a switch inside the cabin, and from the switch to the positive terminal of the battery with an inline fuse. The fan can be turned on anytime though, so just make sure you turn it off when you turn off the car. It's hard not to since the fan is pretty loud when the car is off.

Looking at it more, this smaller fan looks like it flows more air for similar power. I'm gonna try it too. I'd love to figure out a good auto temp switch for it. Open to suggestions...

I used a 6.5". Put the LED "cooling fan" switch im the glove box.
