Lean Bank One Error

Hello All, after checking AccessPort I found the "Service/Check Engine Soon" light was due to a "Lean Bank One" or "Bad O2 Sensor, I am told that my sensors are good, and that I may have a vacuum leak or a fuel issue. Anyone had this happen to their Evo X? Just curious so I can go ahead and clear this error permanently. Any assistance/pointers are welcomed.

That happens when your fuel trims are over +12.5 for a certain amount of time. That can be from a bad tune, different fuel, FPR hose off, etc
