The number in the fuel map is just that, a number.
Yes sometimes it can correlate fairly close to the actual measured AFR, however it is still just a number and should be treated as such when tuning.

In closed loop things are bit easy, due to many sensors and primarily o2 sensor data the ecu can understand the combustion result and trim the afr as close as the table, but that trim takes time too. Check stft and ltft for more info about it.

In the open loop state, there are many factors behind of it. First of all in open loop they simply became numbers like benja said because the data from the sensors are not in concern, so what you wrote in the tables are simply what u get as a guide nothing more. Thats why you should observe the afr and find the exact number for your desired AFR.

At extreme points like you said, there might be mechanical imposibilities also. In instance, writing down 8 at high rpm and load, may result in 12 afr because your injectors capacity and/or fuel pump and/or fuel line might be the bottleneck.

At that point the tuner should know and tell you what to do. As a tuner, generally its easy to see where is the bottleneck due to the log data and the experience of the tuner.
