Yea Swain is the only company that would never do it on the inside (giggity) because of the material they use. The companies that use the paint stuff (like the ones I listed above) can do the inside and generally cover the flange surface. It wouldn't make sense to use the swain material on the flange but the ceramic paint stuff seems to be OK. I may still opt for not doing my flange surfaces...

There are differences in ceramics for sure. As already discussed, some cannot be used inside as they won't hold up. Coating the inside is a good thing as it will help reflect more heat from the manifold/turbine from the inside.
The ceramics we use are the only ones that Garrett themselves certifies for use on/in any turbo application for the RX-7 (one of, if not the, hottest temp platforms).
As far as the flange coating goes, it's really a mute point. We choose not to do it to aid in any potential issue with gasket sealing. We take the time to mask them off and ensure they are ceramic free. Others don't take the time to do that, but that doesn't mean you will automatically have sealing issues.
