throw out bearing fail
After having a shop sink lots of work into my VIII trany; new ACT clutch, plates, flywheel & insert, throwout bearing, and other bit such as syncro's, I put about 250 miles on the car it suddenly developed a loud rattling noise clutch out. I took it back to the shop and they had the initial diagnosis that the new throwout bearing had failed.
The shop called me today after getting grief from the TB makers and asking me if I had been "launching" the car, that the clutch was going to have had gotten very hot. I'm 43 and had actually been taking it a bit easier than usual on an unproven trany. I don't do launches. I drove the original trany well over 100k miles before the original clutch broke.
The ACT clutch has taken some getting used to; the trany action is smooth, but the new clutch feels notably lighter than the and doesn't seem to hook up as crisply as the old one. The shop tells me that it needs to be that way to last. I'm starting to wonder if the shop is setting up the trany as they should.

Sometimes throw-out bearing's are just noisey and that's the way it is. Mine makes all sorts of noise on decel. Almost every car I've had has made SOME noise idling in neutral.
TOB can be noisy..but usually after time and wear. For instance, mine started rattling because it was getting worn out and about 5k miles later the clutch started slipping.
However, in your case it seems that the TOB is having some issues even though it is new.

This one clatters, cold clutch out in the garage, rather loudly. The sound level goes down once it warms up, but is still there. Throw in the clutch and it goes away. In gear the noise persists, drowned out somewhat by the other noises of the drive train and motor.
What bothers me is the call from the shop talking almost accusatory about the clutch getting hot, telling me that there was evidence of that; either on the clutch or having the TOB fail. I drove the original to 116k miles, I think I'm not abusing it.
The TOB manufacturer wants it back, they seem perturbed that it failed. In the next day or so when I get the car back, I'll know something more about what happened.

yeah my act TOB was crap, started making noise a cpl weeks after turbo trix installed it and only goit slowly worse. I have an oem one now, its not silent but much better!
