I've ran turbo'd and NA on the same sensor with both the stock and RRM header. With over 50k miles like that the stocker is still going strong. Maybe it was damaged in removal/re-install or it may just have been a bunk sensor. Throw the stock header back on and have the dealer waranty the sensor... as long as they never find out about the RRM header your golden.

probably fouled. dunno how it would generate high voltage otherwise. if you didn't put the header on right(inside nuts first then work your way to the outside ones) you could have caused a lean leak and the ecu could have just been dumping all kinds of fuel.

That's a good point... check for the exhaust leaks around the flange and pull out the O2 sensor. Give it a good cleaning with some break clean and a toothbrush. Get all the carbon off it you can. For 10-15 bucks mitsu has the header gasket. If you want just replace it new and torque from the center out. Lastly reset the ecu and let us know how it works out.

Doesn't sound like the sensors bad it sounds like there is a condition happening it doesn't like.

that probably is it... if your leaking prior to the sensor it will read super lean and give you the higher voltage readings.
