Thank you for your help and it took me a few minutes to figure out how to change from "Original" to "Patched," which is done simply by going to EDIT>INCREMENT.

I suspect the problem resides in how Tephra 88590715_v7 applies the rear O2 patch for running the wideband, which perhaps uses a different address. Can you provide your XML code for the Rear 02 patch that allows you to run the wideband. I probably need to go through the original thread for Logging AFR through the rear O2 sensor and verify mine is setup that way since that was the basis for MrFred's mod to work correctly.
I didn't even understand how it works, but judging by the attached Excel file, you also need to change - "Front O2 (ADC 04) RAM variable" to 6A6E

Min Coolant Temp To Use AFR Table to 70
you need calc and set:
"Wideband AFR Correlation 1/mL Value", for LC-1 1/mL = 31.9
"Wideband AFR Correlation bL/mL Value", for LC-1 bL/mL = 289
"AFR Table Multiplier High Word" (try set 3) and "AFR Table Multiplier Low Word" (try set 0xACCD) or reload my patch with "modern AFR Table Multiplier" (set to 1)

I use the PLX, which is 10-20 AFR so my settings are 25.5(1/mL) & 255(bL/mL). My other settings are...

I've tried every combination and iteration, but the patch is not working on my 88590715_v7 Tephra ROM. The wideband works, but the front O2 signal just stays at 0.8V and I believe this causes my STFT to pull ~20% fuel and the car subsequently then runs 19:1 AFR and wants to stall out.

If you still have not achieved monitoring of the WB in Evoscan, then what does the patch have to do with it? I have updated the instructions ..
change - "Rear O2 (ADC 0A) RAM variable" to 6A6C
change - "MUT12 Value (To Read Wideband)"to 6A6D ( if you want log wb by MUT12)

I start the car and the O2 signal is just a flat 0.8v and once the O2 Trims are activated it immediately pulls the maximum amount of fuel because I assume it thinks the engine is rich (0.8v). Subsequently, the AFR starts to lean out and will eventually go to 20:1 and the car almost dies because it is so lean. It is very possible I have done something wrong, but this same occurrence was reported by multiple people within this thread. I would be happy to send you my ROM and tune file if that helps.

Request 12 is what I logged with EVOScan and is labeled "Wideband Analog Signal" in the graph provided. I confirmed in the MUT Table that MUT12 is 6A6D.
