do i need a new clutch or just a clutch cable?!?!

hey everyone i have been driving my car around for a while now with the clutch slipping. in every gear if i gun it the rpms shoot high then fall down to a lower rpm then i fell my clutch grab. its been getting so bad to the point where my car doesnt move until i rev it to at least 4500rpms. any suggestions please? could it be the clutch, flywheel, cable or just an adjustment?
any suggestions are appreciated
thank you all
-Chris

there's plenty of threads on clutches and flywheels and evo's don't have clutch cables, it's a hydraulic system.
