year: 2011
first sold: 7/24/11
pump started to fail: 23,000 miles-near 100 degree temps
pump failed: 23,000 miles [pump is being replaced]
cold-start reflashed: never

Update: Mitsubishi will not cover the pump because it has a tune on it! I am so ticked.

YEAR: 2010
1ST SOLD: DEC 2009
PUMP STARTED TO FAIL: ~28,100 MILES (AUG 2013)
PUMP FAILED: ~28,300 MILES (AUG 2013) (don't think it's 100% dead yet, but dealership says it needs replaced - diagnosed 2 weeks after "4WD SYSTEM SERVICE REQUIRED" dash warnings)
COLD START REFLASH: YES, TSB-SC11-002 APPLIED AFTER PUMP WAS ALREADY FAILING. (requested all applicable TSB & ECU updates in DEC 2012 and other visits prior during 3-year warranty, invoices stated everything was up-to-date).

* Dealership will not cover ACD pump under Mitsubishi 5-year/36,000-mile powertrain warranty (out of 3-year warranty by 8 months, though still under mileage). Currently in arbitration with corporate. More info here, if needed (LINK).

* 100% covered & already repaired (not covered by warranty, but Mitsu Corp goodwill according to dealership / covered due to low mileage according to Mitsu Corp)

pump started to fail: approx 22,500 miles, 12/2011 on a week of below freezing temps
pump failed: after i brought it to the dealer and reflashed, they said it needs to be replaced
cold-start reflashed: reflashed when i brought my car to the dealer after the warning light came up, didn't resolve the problem.

*i knew my warranty was up, but i didn't mention it at first. my dealer was willing to replace the pump under warranty, then he called me again and said that my warranty just expired.

* i asked my dealer to price match the pump that's being sold by mitsubishiparts.net ( $350 cheaper ), they did.

Mitsubishi powertrain warranty would not cover dealer said they had seen it before and thought it was butt cheeks; however, car is 100% stock and I have a solid 100,000 mile 3rd party warranty which covered replacement in full. Will rebuild the pump after I determine cause of failure and dissasemble jic I need it later.

Year: 2008
First Sold: ~ 2008
Pump failed: Dec 2012 / 30,000 miles
Cold start re-flash: none (dealer says they have no flash/update for me)

Motor brushes burnt and blocked, probably because of the ticker oil during winter. It was winter's lowest temperature.

Pump started failing: Dec 2013 - 40000kms

Confirmed that it is not covered by warranty.

Pulled the relay out before pump motor seizes while I am ordering the rebuild kit from acd tuning.

Edit: For those wondering which one is the ACD pump relay:
